A simple avatar, a circular head above a wide dome-shaped shoulder line, carries a circled plus badge at its lower right corner. The combination reads as add contact, invite a member, follow someone, or create a new account, which suits address books, CRM and admin panels, team management screens, chat and forum apps, and signup buttons.
The badge overlaps the shoulder and is knocked out of it, so the plus stays readable against busy avatars. Use it next to user remove, user edit and user group icons for a consistent set, and keep it at 20 px or above so the plus does not close up.
