About the System Error black outline icon

A monitor or screen frame sits behind two overlapping marks: a rounded warning triangle with an exclamation point breaking over the top edge, and a toothed gear centered on the lower edge. Together they say something in the machine has failed, which fits error and crash screens, maintenance notices, server status pages, IT helpdesk articles and app diagnostics.

Both badges are knocked out of the screen with a clear gap, so the three shapes stay separate. Pair it with warning, settings, server and repair icons; use it at 32px or larger, since the gear teeth and exclamation mark are the parts that carry the meaning.
