About the Form black outline icon
A single sheet with a folded top-right corner carries a short header dash and five ruled text lines, the last one followed by a check mark. It reads as a completed or approved form rather than a blank page. Use it for online form builders, survey and questionnaire screens, job and loan application flows, checkout address steps, and compliance or audit checklists.
The folded corner marks the top edge while the text lines run at equal spacing to a common left margin, so the page stays clearly a document at any size.
